This page provides 2024-2025 Health Occ students with some additional information and items to be completed prior to classes starting in the fall. There are some items to print or copy and bring to class.
Physical Form - All students should have a physical completed before school starts. A complete physical or a sports physical is accepted with an immunization record. Students may use this or use a form provided by their doctor.

!Respiratory Clearance Form!- **Students in all Programs will need a Respiratory Fitness verification** Form must be signed by your medical provider and is to be completed for all health occ sites.
First Day Instructions - This link gives you class times, where to park, and where to meet your teacher. Each location's instructions will be different and are posted on this page. Your school will give you your location and class section. Please check this page a few days before class starts for any updates.
