This page contains important information and forms that must be completed before the first day of class for students enrolled in the Health Occupations program.

Please review the steps below carefully. Some forms must be printed, completed, and brought with you on the first day of class.

Step 2: Review the Student Checklist

Before the first day of class, students should review the checklist below to make sure all required items are completed.

Step 3: Complete Required Health Forms

All Health Occupations students must complete the following health forms before the first day of class.

Physical Form

All students must have a completed physical exam before school starts.

A school physical or sports physical is accepted, along with an immunization record. You may use the form provided below or a form from your healthcare provider.

Respirator Clearance Form

Students in the Health Occupations program must be medically cleared to wear an N95 respirator, which is commonly used in healthcare settings. Respirator fit testing will be conducted during class.

Medical Provider Responsibilities
The medical provider only needs to:

Check one respirator clearance box
Sign the form
Add the date

⚠️ The medical provider does NOT need to complete anything else on the form.

This form simply confirms the student is medically able to wear an N95 respirator.

Please bring the completed form on the first day of class.
