COVID Exemptions

Our healthcare partners strongly encourage all students to become fully vaccinated against COVID 19.  If you are unable to do so due to a religious or medical reason, you may request an exemption.  Exemption forms should be picked up from and returned to your HOCC teacher.  We will send student forms to the appropriate healthcare facility.  The exemption forms below are for our primary hospital sites - other forms may be needed for additional sites and will be given to students during class as needed.

For students taking Health Occupations  - complete the form for the primary site where you are taking classes.  All forms will be turned in to your teacher.

Agreement for sharing health-related documents - All  students need to sign and parents sign
